NRHA and The John A. Hartford Foundation have partnered to compile resources for rural communities and their providers including webinars, videos, and trainings from trusted partners that highlight opportunities to support the expansion of age-friendly care into rural communities.
Connected Care for Older Adults Webinar
Connected Care for Older Adults is an innovative model that uses Community Health Workers (CHWs) in partnership with primary care providers (PCPs) to improve care for frail older adults in rural areas. Through Connected Care, CHWs trained in Age-Friendly Health Systems conduct a series of home visits with patients and families and implement Connected Care protocols based on the 4Ms.
The Connected Care for Older Adults webinar was presented in partnership with Oregon Health & Science University and the National Rural Age-Friendly Initiative, a collaborative effort led by the National Rural Health Association with support from The John A. Hartford Foundation dedicated to advancing age-friendly care across rural communities.

"Before I Found PACE" is a video series supported by The John A. Hartford Foundation (JAHF) and West Health to showcase the stories of PACE participants and caregivers.
The JAHF, in partnership with the Iowa College of Nursing’s Barbara and Richard Csomay Center for Gerontological Excellence, hosted the "Age-Friendly Care in Rural Areas: Implementing the 4Ms in Public Health" webinar, highlighting the Age-Friendly Health Systems (AFHS) initiative and strategies for implementing the 4Ms Framework in rural public health settings.
The "I Love Rural Health: Nemaha County Hospital" video showcases Nemaha County Hospital's implementation of the 4Ms to advance age-friendly care for older adults in its rural community.

Become an Age-Friendly Public Health System Champion
- Participation as a Champion is open to any and all individuals. Champion recognition is achieved by attending or viewing the recording of six AFPHS monthly trainings. The monthly trainings occur on the third Thursday of each month. Recordings of all the trainings can be viewed on TFAH’s YouTube Channel.
- Individuals should complete the following survey after attending a live training or viewing a recording: AFPHS Training Completion Survey. The AFPHS Team will notify individuals of recognition upon completion of the six surveys.

The Oregon GWEP Free Online Training Program for Healthcare Professionals and CHWs
- Developed by the Oregon Health and Science University (OHSU) partners and based on the Connected Care for Older Adults protocols, this free training is designed to equip Traditional Health Workers (THWs) in Oregon to better support older adults in their communities. Course modules cover:
- Working with Older Adults
- An Introduction to Age-Friendly Health Systems and the 4Ms
- System Navigation for Older Adults
- Community Resources for Older Adults
- Incorporating CHWs in Age-Friendly Care
- The course may be submitted to the Oregon Health Authority THW Program for 8 continuing education credits. For questions regarding the training, contact gwep@ohsu.edu.
Medscape Videos: Age-Friendly Care in Action
- Medscape has released a set of training videos, "Age-Friendly Care in Action," for health care professionals to earn cost-free Continuing Education credits.
- The goal of this activity is for learners to be better able to establish age-friendly care in their clinical practice to ensure that older adult patients have the best possible outcomes.

Oregon Health Authority's Home Visitor Safety Guide:
- Read the Oregon Health Authority Home Visitor Safety Guide, which the Connected Care for Older Adults program uses to deliver a 90-minute internal training for CHWs on home-visiting safety, based on the guide's tips and best practices.
