Let's discuss age-friendly plans for RHTP funds

National Rural Health Association and Stratis Health are excited to invite you to participate in a discussion forum and idea exchange regarding opportunities to leverage Rural Health Transformation Fund implementation to support improved health and care for older adults in your state. This event is designed for Rural Health Transformation Fund leaders at a state level to identify and discuss incorporation of age-friendly strategies as part of your CMS-approved RHTF plans. Whether you are with a state office of rural health, a state Medicaid agency, a state rural health association, or will have a role or influence in how your state’s RHTF will be implemented, if you are interested in supporting age-friendly approaches, this is the forum for you to listen, learn, and share!

What Participants Will Experience
  • Hear from and share with other state leaders ways in which age-friendly strategies are or could be included as part of RHTF plans
  • Discuss opportunities and mechanisms for ensuring that RHTF activities incorporate strategies that address the needs of older adults in rural communities
  • Identify tools and resources that support incorporation of age-friendly best practices into RHTF activities


This free event is supported by The John A. Hartford Foundation as part of the NRHA’s National Rural  Age-Friendly Initiative.

There are two opportunities to participate. Register for either of the sessions listed below by clicking on your preferred date.

  1. Thursday, December 4th from 12:30 pm – 2 pm ET
  2. Monday, December 8th from 2 pm – 3:30 pm ET
